Case Study 4 – Harry Hunter

 

Harry Hunter is a 77 year old gentleman living in the community. Harry is receiving palliative care. He has recently made the decision to stop all medication except pain relief and is currently self-medicating morphine elixir. Harry lives with his wife Esme, who is his primary carer. Your role is to provide respite for Esme once a week for 3 hours. During this time your tasks include assisting Harry with his personal hygiene and offering time to engage in an activity of Harry’s choice. Harry’s interests are currently focused on sorting out his stamp collection He wishes to put them all in albums in date order.

You arrive at 9am to find Harry sitting in his arm chair. You greet Harry and ask him how he is. Harry tells you he is in a lot of pain, and he doesn’t think he can get out of the chair today, to have a shower. He then asks you if you could go and get the morphine from the bathroom and pour him a 5ml does which would be an extra dose to what he has been prescribed.

You do not like to see Harry in pain. You discuss his need to contact his GP for a pain assessment, and possible increase in his medication. You acknowledge that he does not feel like having a shower today because of the pain. You go to the bathroom, bring his morphine out, pour him a 5ml dose and leave it on the table for him to take.
