Maintain a journal of your experiences while you are on placement.

Keeping notes about your experience may assist you to review your personal beliefs and attitudes, your experiences and what you have learnt in the course. This is an opportunity to consider your personal strengths, your limitations, your aspirations and your interests.

Use the templates to guide you through each topic. All templates need to be completed.

How to reflect

  • Think about the situation. What specific tasks did you complete? Try to focus on a certain task and write notes detailing how you did the task, how it may have challenged you and how it made you feel.
  • Evaluate what you did. Was the outcome as you expected? Did anyone give you feedback? Would you repeat your actions, or would you do something different next time?

For this assessment you are required to demonstrate the following skills when working with people with a disability during your workplace placement:

While on your placement you are required to reflect on your experience of working with different people with a disability. This unit is focussed on facilitating empowerment, so please choose an experience which shows these skills. The questions ask you to provide some general information about the work you are undertaking during placement, as well as some specific information about your interactions with clients.

Your responses to the reflective questions must be based on the experiences you have had while undertaking placement. Please refer to client information with discretion. You must observe confidentiality requirements.

Assessment Task 5 – Observation Workplace Assessment

 

During your practical placement you are required to demonstrate your ability to perform a range of personal support activities in the workplace. Different roles and different workplaces will require you to perform different personal support tasks. During your placement your assessor will visit you and observe you performing three different personal support tasks from the following list:

  • bed bathing
  • dressing, undressing and grooming
  • eating and drinking using appropriate feeding techniques
  • oral hygiene
  • shaving
  • showering
  • toileting and the use of continence aids
  • using aids and equipment including devices used by the person
  • transferring a person between bed and chair
  • transferring a person in and out of car
  • falls recovery.

 

This observation assessment will be completed after you have had the opportunity to practise the skills in the workplace. Prior to conducting this assessment your assessor will negotiate with you and your host employer which three skills from the list are the most appropriate to be assessed in the context of that organisation.

CHCCCS011 Knowledge and ability

For each support activity you are observed completing you will need to consider and demonstrate where applicable your knowledge of and ability to:

  • review individualised plans to confirm level of support to provide to the person
  • seek support from colleagues and supervisors where required
  • consider the needs of the person which include their physical and cultural needs
  • confirm with the person the support activities to be undertaken
  • perform each task according to all relevant policies and procedures
  • complete the support activity while maintaining the person’s dignity
  • complete any documentation as required.

This observation may be conducted over multiple workplace visits; your assessor is required to note the date that you have demonstrated each point in this assessment.

Your assessor will provide comprehensive notes to describe the context of the observation including; date, client/s involved, activity being observed and other details as applicable.
