Occupational Health, Safety and Environment Assessment Task 2: Risk Management Report
For the purposes of this assessment, you are to assume the role of a health and safety manager in a medium-sized business. In this role, you have been approached by a number of workers who have complained about a specific OHSE issue (you can choose for it to be any issue that has been covered in this Unit). This same issue has also been raised through the health and safety committee and subsequently the CEO of the company has asked you to prepare a Risk Management report regarding the issue to be tabled at the upcoming board meeting.
The report must be completed individually. It should be no more than 2500 words long ± 10%. M a guide to the word limit, words should be counted from the word introduction” at the top of page one to the word ‘Referenoes’ at the last page. Data and/or assessment tools can be included in the appendices where appropriate and will not be considered in the word count. However, the word length of the appendices should be no more than 1000 words long ± 10%.
Once you have chosen the hazard and specific task for your Risk Management Report, you should carry out an initial risk identification in the work area in order to identify the exposure profile of the hazard and the presence and performance of current hazard control measures.
The report should be organised into sections, all content appropriately referenced (as described below), and include:
Executive Summary: A brief overview of the problem as identified through the risk identification, findings of the risk assessment, and recommendations such as for further assessment and for improvement of hazard controls.
Introduction: Identification of the hazard and a brief introduction to the problem at the workplace you have chosen. This section should also include a description of the work organization/job categories, current use of controls (categorized by hierarchy of controls). Include a workplace or site description – site diagram (optional) and a brief summary of findings from your hazard identification safety inspection (walk through survey).
Discussion of hazard: In this section you must demonstrate a thorough understanding of the hazard. including a general review of the particular industry/worker groups most at risk, current state of knowledge about the hazard, known or suspected effects of exposure to the hazard and relevant standards or legislative requirements.
Risk assessment: Describe your risk assessment methodology and findings/conclusion. You could undertake an assessment based on available information or you could assess the problem in-vivo. You may use any risk assessment methods discussed during the course if these are appropriate.
You should include photographs and any data collected in your assessment to help describe the hazard and task being assessed. You are not required to carry out actual monitoring of the hazard, for example monitoring of airborne contaminants, noise. etc. However, you must demonstrate in the report that you have a thorough understanding of what appropriate monitoring method/s, standards and strategies are such that you could engage a specialist to conduct this work for you. For example, if air monitoring is relevant to the hazard you have chosen, you should describe what type of monitoring (personal and/or area monitoring), what you are monitoring for and the relevant exposure standard/s. Or if you have chosen noise as a hazard you should discuss how noise exposure is monitored, etc. Detailed data and discussion on assessment tools/methods can be included in the appendices where appropriate and will not be considered in the word count for the main body of the report.
Recommendations: You should demonstrate an understanding of the hierarchy of controls as applied to the hazard and recommendations of appropriate changes to eliminate/minimise risks to health and safety so far as reasonably practicable. This includes discussing all control options even if your risk assessment concludes the risk is insignificant. You should provide some brief detail about the recommended control/s. For example, it is not sufficient to simply state “use PPE’, instead you should describe the type/s of PPE, or if recommending local extraction ventilation (LEV) provide some detail as to the type of LVE such as open face or hood LEV. You should also include recommendations regarding further risk assessment/specialist assessment where relevant. Refer to empirical evidence, WHS standards and/or legislation to provide support for your recommendations.

As OHS practitioners are expected to have evidence-based practice, it is expected that you include references from scientific literature and do not use secondary intemet sources Some of these sources from the scientific literature should have been published within the last five years. Specifically, do not use or cite from Wikipedia or internet sources that are secondary or derivative in nature. This suggests that you have poor research skills and that you do not understand the nature of objective, independent information. In addition, do not cite from the Unit Guide.
