You are the registered nurse caring for a patient on their return to the hospital ward following surgery. You will be provided with the clinical scenario on the Interact2 site for this subject. Apply the clinical reasoning cycle to this scenario at the point in time in which you have assumed nursing care of the patient, to establish one priority goal of care. Use the headings of the clinical reasoning cycle to guide your thinking.
The following book chapter will guide you through each phase of the clinical reasoning cycle. It can be accessed via your subject Interact2 site.

For this purpose of this assessment, not all phases of the clinical reasoning cycle need to be responded to. The phases of the cycle that you need to discuss and their suggested word count are:
Phase 2- Collect information (300 words).
Use a systematic assessment tool (e.g. Head-to-Toe, A2K or Primary/Secondary Survey) to collect all relevant objective and subjective data in the scenario.

Phase 8- Reflection (400 words)
- You do not need to include an introduction or conclusion.
- You must use the phases of the clinical reasoning cycle as headings (as provided above).
- Your responses must be written in full sentences and adhering to academic convention (no use of personal pronouns and no dot points).
- Approved nursing abbreviations and acronyms are allowed.
- A minimum of 10 current (less than 7 years old) and appropriate sources of information should be used to support the discussion. - be able to demonstrate a broad factual knowledge base pertaining to health assessment and risk screening maintaining and enhancing skin integrity alteration in bowel and urinary function pre and post operative care and patient education (NMBA 1.1 1.2 2.2 2.6 3.2 5.1 5.2 7.3 7.4 7.7).
